Supporting our Host Home Mentors

You need a big heart to be a host home Mentor. You’ll also need a dedicated team by your side. That’s what you’ll find at Sevita. We’re here to answer questions, provide support, and connect you with the resources you need.

Every Mentor is assigned a coordinator to support them through their journey with us. Your coordinator will provide case management services to make sure the individual’s needs are being met. If you have questions or concerns, simply call your coordinator for support and peace of mind.

We also offer:

  • Skill development and education to help you support an individual in your home.
  • Home visits to check in on progress, offer advice, and help with any need.
  • A network of support that includes experienced coordinators, program leaders, nurses, and therapists.
