Mentor Community Services

For the past 40 years, Mentor Community Services programs have helped adults and children with a wide range of needs, such as disabilities and mental health challenges, live their best lives.

Every person we serve is unique, with different needs, goals, and preferences about the supports they receive. Our teams get to know each person so we can create a plan of service that’s right for them. For one person, that may mean in-home supports and daily social activities in one of our day programs. For someone else, it may mean living in a host home as part of their provider’s family. Our caring team is here to support each person we serve to help them build the skills and independence to live the full, joyful life of their own choosing.

Helping People of All Abilities Live Well Since 1983

Our programs were pioneers of the deinstitutionalization movement that helped bring people with disabilities out into their communities, where they could thrive. We opened our first host home program in Massachusetts in 1983. Back then, there were not many community living options for people with disabilities. Programs like our host homes and group homes have helped people build skills so they could become more independent and enjoy life in their community.

Today Mentor Community Services has programs across the country. Since the beginning, we’ve been driven by the belief that every person deserves to live well. Our programs open doors for people with a wide range of needs and abilities, helping them develop the independence to grow and thrive.

Group Homes

Our group home programs are warm, comfortable places where adults with disabilities get 24/7 support from caring staff. Adults build independent living skills and enjoy fun activities with their housemates and in the community. Our homes are accessible spaces with plenty of room to relax and enjoy hobbies.

Host Homes

In our host home programs, adults with disabilities live with supportive host home providers who help them become more independent and enjoy neighborhood life. Supports include:

  • 24-hour home-based support
  • Community activities
  • Skill building
  • Support with daily living activities
  • Transportation

Day and Vocational Programs

In our day programs, adults with disabilities and other challenges can make friends and build skills for work or independent living, all while enjoying fun activities at the center or in the community. Activities and supports are tailored to each person’s needs and interests and may include:

  • Art and music
  • Cooking
  • Community outings
  • Volunteer projects
  • Job skill building

In-Home Health Services

Our in-home support services teams take the worry out of daily living so adults and children with disabilities, medical challenges, and other needs can keep living at home. Support is tailored to each person’s needs and goals and may include:

  • Skill building
  • Help with meals and housekeeping
  • Support with personal care
  • Transportation for errands and activities
  • Nursing care
