Our caring team members work closely with each individual through all service planning to learn about their needs and what is important to them. We then create a plan of service just for them, with a unique mix of supports that may include residential services, day habilitation, nursing care, behavioral health services, and other supports, depending on their needs.

Group Homes

Our residential homes, also known as group homes, are warm, welcoming residences that can support individuals who need intensive 24-hour care as well as those who are able to live more independently. Services may include support with:

  • Daily living such as meals, housekeeping and personal care
  • Nursing care
  • Community activities
  • Transportation
  • Accessibility needs, such as wheelchair access

We offer several types of group homes including HCS, ICF-ID, Medical Residential, Supported Living, and Staffed Residences.

In-Home Support

Our hourly services help those we serve live as independently as possible in the home of their choice. Services provided in the home may include support with a wide range of needs, including:

  • Daily living such as meals, housekeeping and personal care
  • Medication administration
  • Community activities and outings
  • Transportation
  • Respite care

Host Homes

In our host homes, adults and children with disabilities live in private family homes with caregivers who support them. Individuals become part of their caregiver’s family and receive support including daily living skill development, transportation, and more. This warm and caring family environment builds long-lasting supportive relationships that help individuals thrive.

Day Habilitation

Our Day Habilitation programs provide independent living skill building, social activities, and other supports to help individuals build on their strengths, talents, and skills. Supports may include:

  • Social skill development and activities
  • Nutrition
  • Exercise
  • Job training and coaching
  • Art and music
  • Community activities

Person-Centered Services

In addition to our residential and day habilitation services, we offer a wide range of supports tailored to each person’s needs and preferences, including:

  • Case management
  • Vocational services
  • Behavioral health supports
  • Nursing care and wellness services
  • Companion care
  • Specialized therapies such as physical, speech and occupational therapy
