Sevita (formerly The MENTOR Network) has provided home and community-based health care for more than 50 years. What inspires us? Seeing the adults, children, and families we serve grow and thrive. And with the help of our dedicated team members, they can flourish.

We support more than 50,000 individuals across the United States. We provide the care and services for them to live independently in the communities they call home. And no matter what the challenges — we’re here. Because every person has the right to live well.
