REM West Virginia's home and
community-based services
REM West Virginia provides caring, community-based support for adults and children ages 5 and older with intellectual and developmental disabilities and other needs. Services include skilled nursing, day habilitation (day programs), case management, crisis services, and more. Our team helps each person we serve build skills and become more independent with tailored supports designed around their unique needs.
Skilled Nursing
REM West Virginia provides RN and LPN skilled nursing services for adults ages 21 and older with complex medical needs. Skilled nursing services may include:
- Nursing assessments to determine the level of support a person needs
- 24-hour nursing care
- Monitoring of ongoing medical needs
- Support with appointments, medications, and more ordered by physicians
- Coordination with physicians and other medical providers
Our nursing team provides person-centered care in an individual’s home, giving people the medical support they need and helping them live healthy, more independent lives.
Person-Centered Support Services
Our person-centered support services help individuals with intellectual and developmental disabilities build independent living skills and take part more fully in home and community life. Services include:
- Independent living skill-building
- Support with personal care, such as hygiene and dressing
- Mobility supports
Available support settings may include:
- Home-Based Person-Centered Support
- Licensed Group Home Person-Centered Support
- Unlicensed Residential Person-Centered Support
- Family Person-Centered Support
Supports are tailored to each person’s goals and can be provided in the home, in residential settings, or in the community.
Facility-based Day Habilitation (Day Programs)
REM West Virginia’s facility-based day habilitation services, also known as day programs, help adults build skills, make friends, and enjoy being part of life in their communities. Activities and supports are tailored to each person’s interests, strengths, and goals and may include:
- Independent living skill-building
- Personal care support
- Exercise
- Art and music
- Cooking
- Community activities
Behavior Supports
REM West Virginia’s behavior supports help adults with mental health and behavioral challenges build skills for safe, stable, full lives in their community. Caring behavior support specialists help individuals increase positive behaviors through:
- Adaptive skills training
- Positive behavior support strategies
- Individualized planning based on strengths and goals
Our behavior management program uses the Positive Behavior Support model which provides rewards for positive behavior rather than consequences for negative behavior.
Case Management
Our case management team helps coordinate the medical, professional, and community-based support an individual needs to thrive. Case management team members provide:
- Assessment to determine strengths, needs, goals, and supports needed
- Service planning
- Linkage and referral to medical, social, and other community services
- Advocacy supports
- Crisis support
- Service plan evaluation
We work with the individual and others in their circle of care to create a support plan tailored to their needs and monitor the plan to make adjustments as needs change.
Crisis Services
Our caring team provides support during behavioral or medical crises, including enhanced staffing when needed to help stabilize the situation and protect each person’s safety and well-being.
Respite
Our respite services give primary caregivers short-term relief while ensuring their loved one keeps getting compassionate care and support in a safe environment.
Transportation
REM West Virginia provides transportation to and from an individual’s home, day programs, job sites, and approved activities included in their Individualized Program Plan (IPP).
