California Mentor's home and
community-based services

Since 1981, California Mentor has helped people with intellectual and developmental disabilities, mental health challenges, and other needs live joyful, more independent lives. We support people from birth to seniors live well with residential and independent living services, day programs, family support services, early intervention for children, and more. Family Home Agency

In California Mentor’s Family Home Agency program, also known as host homes, adults with intellectual and developmental disabilities live in loving family homes with caring host home providers, who we call Mentors. Mentors provide encouragement and 24/7 caring support including:

  • Personal care: Support as needed with hygiene including grooming, dressing, and other needs.
  • Daily living support: Help with preparing healthy meals, chores, as well as providing a clean and safe home.
  • Skill Development: Support building life skills such as cooking, managing money, and communication.
  • Transportation: To activities, doctor’s appointments, and more.
  • Community life: Support participating in community activities, exploring hobbies, and making friends.

Specialized Residential Homes

California Mentor’s licensed specialized residential homes, also known as group homes, are warm, welcoming homes where adults with complex medical or behavioral/mental health needs get intensive 24-hour support. Services include:

  • Nursing and behavioral supports – Behavioral consultations, medication management, and more
  • Personal care and daily living support: Support with hygiene including bathing and dressing, as well as healthy meals
  • Community life: Opportunities and support in making friends, exploring hobbies, and enjoying their favorite community activities. 
  • Transportation: To activities, doctor’s appointments, and more.

Intermediate Care Facilities (ICF)

California Mentor’s Intermediate Care Facilities (ICF), also known as group homes, are licensed residential homes providing 24/7 support, including medical, rehabilitative, and daily living support services for individuals with intellectual and developmental disabilities and intensive needs. These homes provide:

  • Small, welcoming homes in the community
  • 24/7 supervision and support by caring staff
  • Nursing care
  • Support with daily living, such as meals and personal care
  • Rehabilitation services
  • Support participating  in community life

Day Programs

In California Mentor’s Day Programs, adults with disabilities and other challenges enjoy fun daytime activities in a supportive community center setting. Adults in our programs get support for a wide range of daily needs including,

  • Building skills for greater independence
  • Making new friends and building social skills
  • Enjoying fun activities at the center and in the community
  • Nursing supports
  • Behavioral services
  • Advocacy services

Adults in our day programs can choose from a wide range of fun activities that match their goals and interests, including:

  • Art
  • Music
  • Volunteer projects
  • Community outings
  • Job training and coaching
  • College services for individuals attending community college

Supported Living Services

California Mentor’s Supported Living Services provide person-centered daily living supports for people with disabilities and other needs. Services are designed to help each person we serve live as independently as possible, whether at home with family or in another residential setting. Our caring staff provide as much or little support as needed including help with:

  • Personal care such as bathing and dressing
  • Household tasks
  • Daily living skill development, such as making meals and handling money
  • Being active in the community, including attending events
  • Transportation

Independent Living Services

California Mentor’s hourly independent living services support adults with disabilities in developing independent living skills so they can thrive at home, whether they live with family or in another residential setting. Supports are tailored to each person’s needs and goals and may include skill-building and support with:

  • Household tasks such as cooking and cleaning
  • Managing money
  • Personal care and health, such as with hygiene, medication routines, and safety
  • Participation in community life
  • Social skill development
  • Participating actively in community life
Children's services

Early Intervention for Children

California Mentor’s Early Intervention offer structured therapy for children ages 0–school age who show signs of autism, developmental delays, and other challenges. The goals of Early Intervention are to:

  • Identify developmental concerns in an early stage of life
  • Give the family guidance and support as soon as possible
  • Strengthen family involvement
  • Promote growth and support the child in reaching developmental milestones

Our services use play-based learning and education for families to help children grow and thrive. Most services for ages 0-3 are provided in the family’s home, with school age services provided in school settings.

Family Support Services

California Mentor’s Family Support Services help youth at risk and their families work through challenges such as trauma, involvement with the juvenile justice system, and behavioral and mental health needs. Our team of specialists and clinicians helps families in biological, adoptive, and foster homes build skills and stronger relationships that can support them at home and in the community.
