REM Community Services

For more than 50 years, REM Community Services programs have helped adults and children with intellectual and developmental disabilities, mental health challenges, and other needs live the full, joyful lives of their own choosing.

Our caring team members get to know each person we serve so we can understand their needs, goals, and preferences for service. We work with the individual as well as family, medical providers, and others in their circle of support to create a plan of service just for them. For one person, that may mean living in a host home as part of their provider’s family. Another individual may need in-home supports and skill building in one of our day programs. Whatever the person’s needs and goals, we’re here to support them each step of the way as they grow and as their needs change.

Helping People of All Abilities Live Well Since 1967

We opened our first group home in Minnesota in the 1960s, when there were few community living options for people with disabilities. As leaders in the deinstitutionalization movement, our programs opened doors for people with a wide range of abilities to live their best lives in the communities they called home.

Over the years, our work has been driven by the belief that every person deserves to live well. Today we work in communities across the country, helping people with a wide range of needs develop the independence to grow and thrive.

Group Homes

For adults who need 24/7 support, our group home programs are warm, comfortable places to call home. Caring staff offer the support adults need to build independent living skills and enjoy life with their housemates and in their community. Our homes are accessible, with space to relax, socialize, and enjoy hobbies.

Host Homes

In our host homes, individuals live with supportive host home providers and become part of their family. Providers help adults be more independent and take part in all that neighborhood life has to offer. Supports include:

  • 24-hour home-based support
  • Community activities
  • Skill building
  • Support with daily living activities
  • Transportation

Day and Vocational Programs

Our day programs are exciting spaces where adults with disabilities can make friends, enjoy their favorite activities, and build skills for work or independent living. Activities and supports are tailored to each person’s needs and interests and may include:

  • Art and music
  • Cooking
  • Community outings
  • Volunteer projects
  • Job skill building

In-Home Health Services

With the help of our caring team, adults and children with disabilities, medical challenges, and other needs can keep living at home. Support is tailored to each person’s unique needs and goals and may include:

  • Skill building
  • Support with meals and housekeeping
  • Support with personal care
  • Transportation
  • Nursing care
