How to Apply for a Sevita Support Fund Grant
We want to make applying for a Sevita Support Fund grant a smooth process. We’ll need you to fill out a short application form and send us a few other documents to help us understand and document your needs. If you have questions at any point in the process, send us an e-mail.
Who’s eligible for a Sevita Support Fund grant?
- Employees employed by Sevita for at least six months
- Non-Mentor independent contractors with at least 300 hours of contracted billable time in the past six months
- Mentor independent contractors with at least 90 contracted billable days in the past six months.
How do I get started?
You’ll need to send us all the information below. Please fill out our grant application form and monthly budget worksheet and upload your documents using the form below.

- A short written statement explaining why you’re asking for assistance
- Two most recent paystubs for you and any other adults in your home or your most recent tax return
- Third-party verification of your situation, such as:
- Death certificate
- Family Medical Leave Act (FMLA) approval letter
- Fire report
- Physician’s statement
- Police report
- Documentation of the expense you’re asking for help to cover, such as:
- Basic utility bill
- Funeral bill
- Mortgage statement
- Rental lease
